Hi Tracy! *waves from the northerly part of your state*

I'm sure a vet will correct me if I'm wrong, but I would suspect that
it's entirely possible that the low blood glucose could have been
caused by the vomiting which was caused by something else.
Laura has seen blood glucose that crashes and bounces back
during bouts of flu with her shelter kids (if I'm remembering
correctly).

HOWEVER...the problem cells CAN be so small as to leave the
pancreas looking perfectly normal, yet still be a problem.

So...I think a good course would be the one that was suggested.
After recovery from the adrenal surgury, start weaning Tango off the
pred, and do a couple rechecks. (And of course watch behaviour
closely for a return of signs of insulinoma.)
