Honestly, this does not scream "adrenal" to me.
With the lumps I am wondering if what you are seeing are enlarged nodes. These can come from systemic infections or from lymphoma/lymphosarcoma. At times if these exist long term they may simply be common to the individual. Meeteetse finally died of lympho, but that was 8 years after she showed with popped nodes (which she always had) as a kit; for her they were just a personal variation.
Such dermal and fur changes to the tail can happne with adrenal growths, but when only the tail is involved they may be from other causes that are ermal in origin.
When giving any meds (including herbal ones ) do make sure that there are no medical conditions present that counter-indicate against use.
What tests did the vet do? What were the results?
Obviously, I am not a vet.
