Inflammatory bowel disease is a very specific diagnosis with a very characteristic set of signs. Before I would start him on a potentially immunosuppressive agent, I would make sure that all involved, icnluding the pathologist, agree that this is true inflammatory bowel disease.

Lymphocytic portal hepatitis may be seen in chronic inflammatory disease of the GI treact - it could be Helicobacter, IBD, or other forms of inflammation. It is not a condition that should be the focus of treatment.
