I was wondering along the same lines since sometimes the adrenals don't enlarge much despite disease. I know that some folks strongly shy away from the if-in-doubt-pull-the-left-one, but think I'd either ask one of our vets to try that in this situation, or wait a bit and then have the vet go in again in case things were more obvious then.
Like you, we have had fine luck with vulvas reducing after adrenal surgery.
