Sugar does have some significant problems. the symptoms that you are mentioning sound very much like a recurrence of adrenal disease. Sometimes, if even a small part of a malignant adrenal tumor is left, then it will regrow over time. It can be almost micrscopic to start with!

With the adrenal symptoms, the insulinoma, and the lump on the side, I think that surgery is still the best option to see what is going on in there, remove the insulinoma(s) and check out that lump on the side. And sooner better than later.
