Commercial ear cleaners are extremely painful on a cut. If your vet does not find anything else abnormal with the ear, I think it is possible that he had just developed a more severe case of earmites, making the ear cleaner especially painful. The increased blood flow to extremeties could just be the result of pain and his sudden outburst of activity.

I have dealt with earmites countless times and my vet and I always treated it with two injections of ivomec (2-3 weeks apart), accompanied by daily ear cleaning. To clean the ears, I use a 50:50 solution of apple cider vinegar and water. This is not only less expensive, but it isn't painful. If the ferret is given a treat during the process, they usually don't struggle at all.
