I am not a vet - this is only my experience - please don't take it as gospel.

My experience with urinary tract infection, in both my cats and ferrets, is that it makes them want to pee more frequently than usual, and often unproductively. The infection causes a burning, itching feeling which is easily mistaken for the need to urinate.

Since your guy is withholding, straining, and has problems with his stools as well, I would suspect something other than a simple infection, though not being a vet, I can't suggest what that would be. It sounds like pain rather than burning. Perhaps you need to convince your vet to dig a little deeper than suggesting "an infection".
