Sounds like it's time for listen and a chest x-ray to find out if there
may be lung disease or cardiomyopathy. (There may also be
need for an ultrasound, for instance if hypertrophic
cardiomyopathy is suspected since that is hard to see
otherwise.) Treatment will depend on the cause.

If the breathing is terrible then don't wait -- have an emergency
appointment.
