Dr. Williams added the United ADV testing instruction sheet to the
files section of this site. It includes the address for where to
mail samples. Look for the file named:
unitedceptest.pdf
The Avecon website is listed in the bookmarks section of this site.
At the website, you can print off a form to send with samples that
includes the shipping address.
Since you are in the Western part of the country, and United has
lowered their prices, United would be the most cost effective place
for you to send samples.
