There are multiple possibilities, but the first one I'd check with
that suite of symptoms would be insulinoma. Have his blood
glucose run, if low then put him on Pred while you learn about
and select among the options. (Surgery is best.)

If not insulinoma then I'd go to further testing beginning with
blood tests and chest x-rays.
