I would guess that in cases where Lupron doesn't work, it might be
because the adrenal gland is overgenerating the hormones on it's own
instead of from input by the pituitary hormones that usually trigger
the problems we see. Lupron desensitizes the pituitary and doesn't
act directly on the adrenal gland.

Perhaps age has something to do with this lack of response to Lupron,
but it might also be due to the type of tumor or its location in the
adrenal gland or maybe even its size. Just as insulinomas cause
increased insulin production without any outside influence, it seems
possible that there could be a tumor in an adrenal gland that
directly results in increased hormone output of the gland.
