Actually the most important things in permission to treat letters,
no matter what state you're in, will be your name, address, and
(hopefully) a way to contact you, a statement that you are the legal
owner of the animal and are leaving them in the care of so-and-so,
and that so-and-so has your permission to authorize needed medical
care for your animal in the event of a medical emergency. A useful,
but not always required, addition is a statement that acknowledges
that you are willing to be held financially responsible for any
treatment needed.
