Let a vet know that you can see a tear on the eye with loose tissue. That
should not wait and needs meds so that it doesn't worsen. She may even
have done that part of the injury herself after the exam, though it may have
been before and from something like a cat scratch as you said.

Those should be acceptable ways to put wieght on, ulness she has early
insulinoma in which case the Nutrical may not be appropriate. a/d, which can
be purchased from a vet, is another fine way to add wieght.

Given her age and the weight loss I think I'd have some testing done: a CBC
with Chemistry Panel, a blood glucose level, a chest x-ray (because
hypertrophic cardiomyopathy can cause unexplained cause wieght loss from
our experience and that of something like 5 other people who mentioned
going through that, too), etc. The weight loss is not a large one but she is
boney by your mention so she may be someone with not much to spare, and
these tests are good to run now and then at older ages, anyway, for safety's
sake.
