Nodes on the pancreas can be very small, sometimes visualized only by
exam and sometimes only on biopsy. They can be quite large as well- but
even these are made of tissue that doesn't show up on x-rays, sometimes
can be seen on ultrasound, but depends on the size- any size can produce
the insulin
and therefore cause the lowered blood sugars.

The best thing to do if the blood sugar is low, symptoms and other
exam point to insulinoma is to go for surgery to check the pancreas and
remove part of it for treatment and biopsy.
Also, if that is not an option, or while waiting, prednisone is started.

Lymph nodes ( not sure which nodes you meant) are also small to big
in size, not usually visualized by x-rays,and are just felt on exam, or
seen
when in surgery.
