I think you have room to increase his meds quite a bit, which may increase the quality of his life considerably, since it seems he already has times when he's enjoying life.

"CMPD" is an abbreviation with which I'm not familiar (compounded?), but assuming the rest of the equation is not based on whatever that is, you have quite a bit of room to go up on both drugs. Of course you want to discuss this with your vet, but if the only other choice is euthanasia, I believe it is worth a try.

It's what I would do.
