If insulinoma is advanced then added sugar will help, not hurt. We've even had one get 3 extra good months with that trick after medications didn't work any longer.

He's eating way, way too much.

If the protein aspect bothers you then give frequent TINY amounts -- really tiny.

If he's going to lose weight he will need to be taking in fewer calories than he is burning. Right now he is taking in way more calories than he can burn so he is continuing to gain weight.

If his fat levels are so bad that they are interfering with breathing or other essential functions he is morbidly obese and his weight itself is a very serious threat to his health.
