I would definitely get him back to the vet. It's possible the
antibiotics didn't fully knock out whatever your vet found and he may
need another course of antibiotics, possible with a different
medication. If his only symptom is diarrhea our vet normally wants a
stool sample rather than blood work. Blood work would only come if he
couldn't find anything in the stool and the problem didn't respond to
his conventional treatments.

We've dealt with a number of problems that caused diarrhea: Pertwee had
heliobacter, Adric and Romana had giardia (we had to threat all our
ferts for that since it's contagious), etc... None proved serious but
the vet's diagnosis did determine the course of treatment, and it was
different in different cases.
