Hi Brandi,
Sounds like you have several questions about Sam,
so I will try to answer them all. If the adrenal glands
and prostate were enlarged on the ultrasound and he
is having urination problems, then surgery is the best option.
However if your vets are unwilling to do surgery perhaps you
should check with the vet school in Berlin, if that is possible.
If that is not possible then what I would recommend for
medical treatment is Lupron depot (leuprolide acetate),
Propecia or Proscar (finasteride), and an antibiotic (Baytril
enrofloxicin). Lupron will stop the stimulation (LH) to the
adrenal glands. Thus they will stop overproducing the
adrenal sex hormones.
Propecia/Proscar will stop the testosterone from being
converted to dihydrotestosterone (DHT). Dihydrotestosterone
is the hormone that is believed to cause the prostate to
become enlarged. Without DHT the prostate shrinks.
Baytril is a good antibiotic that reaches the prostate and is
good against the likely bacterial infection.
Melatonin is also a good product. There are actually melatonin
receptors on the prostate. I would use it with Lupron, Propecia,
and Baytril. The recommended dose is 1mg of melatonin
orally about 8-9 hours after sunrise, or to use the male mink
melatonin implants every 4-6 months.
