I would try to rule out endocarditis too. It can cause the fever and if the
infection is within the sac of the heart it may not respond well, was he
ultrasounded? Also there are other causes including sepsis but if you don't
find the source you may lose the war for sure. If all other options fail I
would consider exploring him surgically that way you can see if he has a
peritonitis (which as was suggested could be secondary to a foreign body).
I also agree with you you have to do something to figure it out or I am
afraid you will lose him for sure. Good Luck and hope you find someone who
