I can think of a couple things to consider, bad tooth,
infection, kidney problem or metabolic problem causing unusual
odor ( like in diabtic ketoacidosis there is a sweet smell, like
when someone has been drinking hours before)

I would treat for the ulcers just in case with carafate before meals
and bland diet ( meat baby food will do for now)
Get him a basic blood test- kidney, glucose, and liver panel
and a CBC. Go from there. He may improve with some Sub Q fluids
as well.
