Hi Yvonne,
The x-rays suggest juvenile lymphoma with
a mediastinal mass. The prognosis for juvenile
lymphoma is usually poor, and with the signs this
boy has prognosis is definitely poor.
A regular CBC may pick up the atypical lymphocytes.
An aspirate of the pleural effusion may also pick
up the atypical lymphocytes. Either would give
a strong tentative diagnosis of lymphoma.
It is time to consider a biopsy to confirm if you
are considering chemotherapy, or to start on
pred to see if you can get any improvement.
