If 0.2 "mil" means milligrams, that's a pretty small dose. At 3 years, surgery is going to give you the longest amount of symptom free time, but in the meantime, I would ask your vet about increasing that dosage amount. Particularly if you're still seeing symptoms of hypoglycemia.
