Honestly, I'd get to the vet. A biopsy would likely make sense to look for Inflammatory Bowel Syndrome and Eosinophylic Gastroenteritis, as would looking to see if there is a shifting partial blockage in the stomach and removing the blockage if present.

One short cut would be to look up names like Rinky Dink or Sherman if seeking IBD help since I know off-hand there is info there on treatments like Prednisolone, antibiotics, Imuran, injectible steroids, special foods, etc. The reason why special foods are needed is because sometimes an allergy goes along with IBD. Many with the problem have difficulty with chicken; we've had one who could not tolerate any type of poultry, and Rinky Dink was especially hard to work up a diet for but they managed it and learned a lot worth reading.

They DO strain with diarrhea because there is nothing to press against, and they get rectal prolapses doing so.

I'd really want to make sure with this one that there isn't a shifting blockage or a tumor, and to try to get a handle on what disorder it might be if this were one of our's.
