You should be able to find an eardrum rupture on exam but usu they need to be undera anesthesi for a good look. If there was a perforation and bacteria or even medicaton got into the eardrum it is possible, but I think it would need to be punctured by ear cleaning etc. Since she is young be certain no mites.
