Usually you'd start to think about the R adrenal; that would be the most likley scenario.
Less common would be bladder stones or infection. Those could also happen in conjunction with adrenal growths if he isn't emptying well for long enough, and infection can be caused by bladder stones, as well as a type of bladder stone being able to caused by infection.
