That blood sugar is low enough to perhaps be the cause of some of the symptoms. Since the vet doesn't want that much Pred in while fighting an infection what about having a small amount of Diazoxide compounded? You can find compounding pharmacies in the Reference Shelf within the Files section in

103 is not a fever. It's kind of borderline for their normal range-- high for normal but not a fever.

How long does the neck torsion last and is it severe?

Curled feet are something I have also seen with muscle loss in some severe malignancies (lymphoma, carcinoma), and I think in one who had cardiomyopathy when it was advanced. Massage of the walking surface helps relax them. They could also have a neurological cause.

Honestly, I have no idea what is going on with this one, so I am just kind of rambling with what comes to mind.
