CLINICAL INFORMATION:
There was acute onset of cutaneous and subcutaneous swellings. These
appeared to be very painful to touch. On an aspirate, there are highly
vacuolated cells with prominent nucleoli.
MICROSCOPIC:
Submitted are two biopsies from the flank and dermal mass from the
dorsum. Both of these biopsies are similar and will be described as
one. One biopsy is of haired skin in which there is extensive inflammation
and necrosis of the mid dermis and extending to the level of the hair
follicles. The inflammation is of a mixed call type with lymphocytes,
plasma cells, neutrophils, and moderate number of macrophages. These
macrophages have a fine granular amphophilic cytoplasm. This inflammation
is resulting in extensive necrosis of the mid dermis with abundant edema
and fibrin deposition. The inflammation does extend to the deep border of
the biopsy section that is of haired skin. In the biopsy section without
natural borders, this same inflammation is present effacing the adipose
tissue as well as partially effacing skeletal muscle and extending between
the muscle fibers.
