At that age and with those symptoms what you need to do is to have a vet appointment.
Check for insulinoma.
Check for cardiomyopathy.
Look at other possibilities, too with things like a CBC with Chemistry Panel.
When you know what it is there IS a LOT of info out there on how to treat these things.
