A biopsy would make sense to make sure he doesn't have Eosinophylic Gastroenteritis.

IBD (Inflammatory Bowel Disease) is a possibility. Change foods to ones with NO poultry, especially no chicken. If that gets an improvement It may be okay to try foods that have turkey but no chicken to see if turkey is tolerated. When a food allergy is part of the problem it is usually chicken, but can be other foods. We've had one who did not teolerate any poultry unless it was well broken down (as in Feline ZD) so he normally ate mammal meat, and another FHL member had Rinky Dink who only tolerated Canine ZD (with supplementation) and ham baby food. For IBD antibiotics sometimes help but it is not unusual to need steroids or even Imuran. Some are helped by having canned unsweetened pumpkin added to the diet, but that does not work for some others.
