You have the relationship with your vet and are comfortable with them so I
usu say stay with them. Exception is if you feel someone else is more
qualified or you are more comfortable with someone else then that is your
choice.

As far as surgery goes, I would be sure you resolve the other problems
before you consider surgery and then reevaluate. You also assume that at
some point there will be problems and if you do surgey when it is smaller,
esp on the right you have increased chance of complete removal and can
assess all other area of the abdomen. Was the IBD confirmed with biopsies?
If not this might be a good time to do that too. I would be very concerned
about the difficulty breathing and be certain you fix that first. How is
the heart? Did you ultrasound that at the same time? I also don't expect
to see fluid in the chest with pneumonia, so is the diagnosis accurate? Or
is the fluid you mentioned in the abdomen? If that is the case it might be
from cancer, and might even indicate metastasis.

Too many more questions for me to give you a great answer, but hope this
helps give you direction.
