The insulinomas are just to small to see and not there on the surface of the pancreas. Even if your vet did find a tumor or two on the surface, it is likely many smaller (maybe even microscopic in size) tumors are seeded throughout the pancreas.

This is why a partial pancreatectomy is usually considered the best surgical route to go when treating insulinoma. Removal of a large portion of the pancreas increases the chances of removing many of these unseen tumors.
