I am not a vet. The pic looks like you have two problems. That is
definitely a swolen vulva. It may be from her remaining adrenal gland
and hopefully the melatonin implant will shrink it. The other thing is
most probably an abcessed anal gland. You can try antibiotics, but most
likely it will require surgery to clean out the remaining fragment of
the gland. Discuss it with your vet.
