One of mine was on something like 4-5 mg/kg every six hours for a little while.

If the owner is comfortable with the idea of giving sq shots, it might
be worth a try to switch to the injectable form as a trial. I had one
guy, Pumpkin, who did a lot better on the shots than on the oral at
the same dose. He got 3 shots a day for something like a year or year
and a half and did great until he very rapidly went downhill in spite
of increasing doses. I used really small insulin syringes and needles
and gave him the shots while his nose was buried in chicken gravy or
ferretone, and he rarely even noticed. Worth a try... Someone else
had another case long before Pumpkin that was similar.
