That DOES sound like it was a bad batch or a batch that a distributor handles wrong causing it go bad.
Yes, that sort of thing has been heard of before with several types of vaccines and manaufacturer usually follows up on the batch, warning all who have it once they know. We have posted batch numbers along with reports when such things have happened with other meds and vax in the past, so have some other lists, but it has been a very long time (years) since the past such report, I think.
