Hy Marie,
You have indeed listed the 2 most likely causes
(spay that left part of the ovary or adrenal gland disease).
Either way surgery is the best option. When she first
goes into heat would be the best time for the surgery because
if there is part of an ovary in there (when she goes into heat)
it will be big enough to see and remove. If there is no ovarian
remnants, then check the adrenal glands. I have removed
ovarian remnants (and cryptorchid testicles) before and it is
a relatively straight forward surgery.
