I hate to read things like this and I hate to have to write this, but with this sudden onset and the pain I would be absolutely sure of doing a CBC with Chemistry Panel before even considering surgery.

Sudden and extreme onset is something we have seen twice. Neither had true insulinoma. The first was a 6 year old who had the symptoms like that because she had lymphoma in her pancreas, and the second was also 6 but in her case it was carcinoma based in the pancreas which is even more virulent than lymphoma.

Hopefully, it will be only insulinoma, but you need to see if there are other values off because that could be an indicator to not risk surgery for this little one, and to perhaps start with higher Prenisolone doses than you would for insulinoma. Also, if the liver values are off that is a good indicator that you want to be sure to use Prednisolone instead of Prednisone.
