Black blood in feces (black or very dark feces), grinding, improving with Carafate and antacid are symptoms of ulcers.

Frank (red) blood in stool if that is also present culd indicate colitis or another lower GI tract problem.

but it sounds to me like those meds and a bland diet (such as meat baby food) may be your best measures. I'm a bit short on time right now, though, so may be missing something and those resouorces will help you fill in what I miss.
