Like Sukie said, maybe he needs more than pursestring surgery. Also, it sounds like the hydro b is hurting him...if he cries and runs when you try to put it on him (maybe it stings). Maybe your vet can give you something else. About his penis...the vet needs to find out what's wrong w/ that too. Maybe he needs to do a culture of the prepuce to see if there's an infection. A urine should be done too...maybe he has a UTI, maybe he has stones. Now, I just realized he's been on antibiotics so a culture can't be done right now. Has bloodwork been done?
