That is going to depend on what is found.

If there is cardiomyopathy then an ultrasound will determine the best approach.

If there is insulinoma and surgery is decided against then Prednisolone with Diazoxide being tried later as things worsen, or both from scratch. A high prtein diet seems ot be best for most until the final stages when adding sugar can sometimes add more quality time, but there have been some individuals who do better with more carbohydrates in the diet.

If something else which is more limiting like lymphoma or carcinoma then there are appraoches for those, too.
