Sure sounds like he has a gastric ulcer. A/D is good, though I prefer plain old Gerber's chicken baby food. Not sure about the puppy milk replacer. I don't see any reason to give him that.

He should be on Carafate 10-15 minutes before feedings. This seals off ulcers, making eating less painful and helps the healing process. A bland food (baby food, A/D) is essential too.
