This is totally without medical authority, but I believe this may be because diazoxide's main action is to suppress insulin output by the pancreatic beta cells. If the insulinomas are functional tumors that are putting out insulin regardless of any controlling factors, diazoxide isn't going to have any effect.

I'd guess it is pretty much the same thing that happens with adrenal disease that is not responsive to Lupron or melatonin. If those adrenal tumors are cranking out sex hormones of their own accord and not doing so due to stimulation by circulating LH, Lupron/Melatonin won't help.
