How enlarged is it? Dangerously so? If not, I might reconsider removing it and try to work on finding out why it is enlarged before diving in.
Sure, a massively enlarged spleen could be compressing other organs, affecting appetite, causing weight loss, etc., and depression due to loss of her home could also be a factor.
