An ultrasound of the heart is the best place to begin.
Typically, there is cardiomyopathy. You can learn some stuff here:

Low blood glucose is an insulinoma indicator if it is low enough. One heart complication that can occur with insulinoma is heart node block. If further testing points to that this will help:

and that, too, should be followed with more recent info from the archives.
For the pain when urinating and defecating your vet has to check into urinary tract health, adrenal health, and possible GI problems. It might not hurt to do an abdominal ultrasound at the same time as the chest one. Urine checks and CBC with Chemistry Panel would probably make sense for this little one, too, I suspect.
