Two possibilities I would consider are infection and hypertrophic cardiomyopathy. There certainly are other possibilities which may just not be showing themselves more yet; for example, we have encountered those symptoms with one sweetheart's thoracic lymphoma case which hid itself despite repeated careful testing; in his case the only test not thought of until later which may have helped find it would have been testing the bone marrow.
For the hypertrophic cardiomyopathy possibility: it is common for that to be found without an ultrasound of the heart.
