Subject: [ferrethealth] Ovarian neoplasia

I=B4m a vet who was asked to look at a ferret as a 2nd opinion. The ferret =
was spayed jan 2005. June 2005 there appeared vulvar swelling. August 2005 =
treated with hCG ( I presume) - 2 injections - swelling went. November 2005=
swollen again, vet did exploratory lap., nothing concrete found, but some =
suspicious tissue removed, sent to lab and identified as fat tissue. Swolle=
n vulva remains. Jan 2006 treated with hCG again, swelling went down a bit.=
Ferret active, no weight loss, no fur loss, appetite fine etc. during the =
whole time. I had a look at the ferret in April. Ultrasound identified a sr=
ucture in the right abdomen,1cm diameter, probably ovary. Laparotomy reveal=
ed a mass, approx 1 cm diam, but with a blood-filled structure attached (ab=
out 1 cm also). The mass was adhered to the colon. Mass removed and sent in=
for Histopath. Came back as :
Soft tissue mass, no recognizable ovarian tissue. Majority is multilobulate=
d neoplasm composed of rather pleiomorphic, spindloid to polyhedral cells a=
rranged as solid sheets, or interlacing bundles, separated into indistinct =
lobules. Cells mostly poorly differentiated, moderate nuclear atypia and mo=
deate mitotic activity, but in some places they are better differentiated, =
with elongated, blunt-ended nuclei consistent with smooth muscle origin. sm=
all nests and cords of polyhedral cells apparently distinct from the tumour=
cells are scattered throughout the mass. these have moderately abundant, g=
ranular or finely vacuolated, eosinophilic cytoplasm and round nuclei exhib=
iting minimal aniskaryosis. They have the appearance of neuroendocrine cell=
s.
Conclusion - based on histological appearance, history and location of mass=
- poorly differentiated smooth muscle tumour of ovarian origin. the appare=
ntly endocrine cells may be ovarian interstitial cells trapped within the t=
umour.
I checked the adrenals while I was in there, and their gross appearance was=
normal. The ferrets fine and the swollen vulva has disappeared.
I presume there was an ovarian remnant left behind after the first spay, wh=
ich progressed to neoplasia, am I right? Did the hCG treatment have any bea=
ring on this? Does this fit with the labs histology report? What are the ch=
ances of it recurring?(think I got it all but due to close adherence with c=
olon, can=B4t be 100% sure). I would like to hear ferret vets opinions so I=
can better advise the owner. How common are these tumours? Thanks.
