It is very likely just caught.

Call back and ask if they used dissolvable stitches.

If they did then what is probably a rough part that is partly dissolved and snagging so should let go soon.

If not, there may be a knot in the stitch that wound up trapped inside.

Retained stitches that don't dissolve can become a focus or swelling or sometimes even infection. This can happen with humans, too, and even with the best of surgeons and follow-up care.
