Is the blood possibly from a vaginal infection or stump pyometra? Some vaginal infections
can be secondary to an adrenal growth (among other possible causes). We had one many
years ago who did that.

Are there any lumps near the urethra like might occur with an apocrin tumor?

Do you have any ferrets there who punish others by biting their privates? We have on
occasion encountered that behavior.

Is there any sign of parasites, fungus, or urine burn?
