what is interesting is it sounds similar to EPSM (Equine
Polysaccharide Myopathy or some sourses say Myolitis) which is a
problem found in horses and one that once diagnosed is often treated
( or prevented) with a change in diet from high carbs to high fats.

both these problems seem to be a major problem of muscle tissue
metabolism that leads to many other problems. Neither are bacterial
or viral in source and both require a biopsy fo r definitive
diagnosis.
Must be horrifying to see your friend go through it!
