I would consider surgical exploratory. I rarely do ultrasound
because it is still not better then seeing with the naked eye. I do
them for non surgical candidates. No matter what others say I have
found things surgically that can't been found on ultrasound and it
ends up costing less if nothing is found. But you are running out of
options. Also if labs haven't been done do them again, NSAID's like
Metacam can cause kidney discease and if this has been going on a
while then there may have been some prexisting issures too. Hope
