Did he say WHY he thought she may be an incomplete spay? Was it based
on the swollen vulva alone?

Yes, it could be the other adrenal gland.

If you are going to have to have surgery anyway, my vets usually tell
me not to spend the time or money for the ultrasound. UNLESS, it is
needed to guide the surgery. While your vet is doing the surgery,
(s)he could look at the abdomen and see if there are any reproductive
remnants remaining. I think there are hormonal level tests that could
be done, but again, it should be resolved by a look around.
Have you used Lupron or melatonin?

The agressiveness towards her may also be a sign of adrenal disease.
